If you don’t have a current résumé, please answer the questions below on additional electronic pages/sheets of paper. If your résumé lacks information on any of the questions, include answers to those questions on additional sheets as needed.
Work History
List your most recent jobs (go back 5 positions or 10 years). Include month/year for the start and end date (if relevant) for each job. For each job, briefly list your most important responsibilities and 2 or 3 accomplishments you are proudest of. If you have relevant work experience earlier than the 5 jobs/10 years limit, include that, too. Consider which skills you have (whether from work or other experience) that could be especially valuable to an employer (e.g. effective public speaking).
Volunteer History
List any volunteer activities. Give month/year dates for your involvement with each organization you mention. Include a brief list of tasks you accomplished for each volunteer experience. Also include any achievement of which you are especially proud or which illustrates some skill you have.
Education and Training
List any degrees, training programs or educational certificates you have completed. For each educational entry, list the school or organization you attended, the dates of the degree/training/certificate, any majors or specializations, any special awards or honors.
Computer and Other Technical Skills
List all software programs you use and indicate your level of comfort with each on a scale of 5-1 (with 5 being “expert” and 1 being “familiar with”). List any other technical expertise you feel comfortable using in a job.
Foreign Language Skills
List any language besides your native language which you can use at a practical level. Describe how comfortable you are with spoken and written use of the language. If English is not your native language, say what is.
Professional Memberships
List any professional memberships or affiliations that are relevant to your objective. Give dates and any offices you have held.
